 The Clinic in Bisbee

                                                              Non-Discrimination Policy 

 The Clinic complies with the provisions of the Civil Rights Act of 1964 and Section 504 of the Rehabilitation Act of 1973, and all requirements imposed pursuant thereto, to the end that no person shall, on the grounds of race, religion, color, sex, national origin, sexual preference, ancestry, age, familial status, veteran status, culture, language, socioeconomic status, gender identity or expression or physical or mental disability or handicap, be excluded from participation in, be denied benefits of, or otherwise be subjected to discrimination in the provision of any care or service. This Nondiscrimination Statement applies to patients.

Specifically, the above includes (but is not limited to) the following:
 
1. Outpatient care will be provided on a nondiscriminatory basis; all patients will receive care without     regard to race, religion, color, sex, national origin, sexual preference, ancestry, familial status, physical or mental disability or handicap.
 
2. All patients will be assigned and/or transferred to exam rooms, in accordance with their medical need and without regard to race, religion, color, national origin, sexual preference, ancestry, familial status, physical or mental disability or handicap, except in cases where patient safety must be considered.    

                                                    Medicare & Medicaid Accepted

 The Clinic is contracted with federal & state governments to accept Medicare & Medicaid plans. Medicaid in the state of Arizona is known as AHCCCS. The Clinic is contracted with at least one AHCCCS plan provider at all times.

                                                
                                                      Discount Application Process

 A completed application including required documentation of the home address, household income, and insurance coverage must be on file and approved by The Clinic before a discount will be granted. If the applicant appears to be eligible for Medicaid, a written denial of coverage by Medicaid may also be required. Adolescent patients seeking confidential care are exempt from the application process, and services are provided at the nominal rate. 

                                                                           Medical

The discount is applied to all in-office services and off-site services supplied by Dr. J. Schourup, The Clinic’s healthcare provider.

                                                                        Lab & X-ray

Reference laboratory tests and consulting radiology interpretations are excluded. 

                

                                                              REFERRAL POLICY

 The purpose of this policy is to for referrals and Continuity of Care. The Primary Care Physician [PCP [maintains a complete and up-to-date clinical record. When a referral is initiated, the PCP provides the specialist with requested information regarding the patient. The PCP informs patient of need, purpose, expectations and goals of the specialty visit; and provides patient with the specialist contact information.
The Specialist provides a consult report for the PCP, which indicates the assessment, evaluation, and recommended treatments. The PCP will discuss this information with the patient to assist the patient in choosing treatment options, and understanding how the specialty information impacts the patient’s health situation.
